I haven't had that happen to me personally, but back in days where me and 6 other guys and 1 girl, where playing BTB.
One game, we had a great amount of lag, i mean really really great, from the beginning of the game (Headlong), maybe even in the pregame lobby. Due to standbyers.
First thing i noticed was one guy from my team was shooting at some of my teammates. After they yelled at him, he said it was because they had the wrong team colour. Naturally he stoped, but he has not able to tell the other team from our team. I only think he saw a couple of players miss-coloured. I don't think he was aware of the symbols on the head like in the video, didn't think about that untill now, maybe there where not there, i don't know.
We didn't care much about it, because the game was lagging like the host used a 56k modem. Things didn't spaw the first few minutes, like the ramp, and that other big thing hanging in the air. The colour blind guy was also missing textures, for a long time, he told us.
It is such a long time ago now, and it may have happened more than once. I actually think the colour episode happen even once more on Zanzibar. In the Zanzibar game, the windmill's blades didn't spawn. And it was the same guy again, he always had the worst connections.
I'm pretty sure it was caused by lag, and i do believe he eventually was able to see the real team colours.

i'm not sure how it works in matchmaking, but in normal games its as simple to do as switching teams while you are dead. At least I've done it a bunch of times while playing zombies. The best part is your radar still works for the other team, and you see the symbols over the other teams head (so you can see where people on the other team are)

That pretty common thing, I've had it where I was mis-colored, I was walking behind a blue team guy (I was on red team) like he was on my team because my arms where colored blue, but then I noticed that the guy had no friendly icon
