I dont think they have released the tags, but maybe if you ask the h2 team nicely

You could model it easily yourself, or edit the existing one and add a first person model.

Ohh, I think it is on the platform above the platform you teleport to above the bridge, so where you teleport to by walking through the teleporter under the bridge (that reminds me of a song..) it is on the bit above where you teleport to i think, it might be on both sides or just on one side im not sure.
