Weapon mods Hall of Fame?

Over the years, there have been some great weapon mods, amid an ocean of crappy ones. Because weapon mods are often just a small part of an entire map mod, I think they can be easily overshadowed or overlooked because of other aspects of the mod.
I'd like to make a list of outstanding weapon mods. I also want to distinguish between skin/model/effects/sound injection mods, as opposed to mods that introduce a new twist on gameplay and functionality. For example, if someone makes an excellent skin and model with custom sounds for the magnum, that's the first category. ExileLord's Gravity Gun, on the other hand, makes almost no cosmetic modification to the stock sentinel beam, yet it is an outstanding mod that belongs in the second category. Some mods may fall into both categories.
A great weapon mod should be almost a new weapon entirely. It should add something to the game that wasn't there before. And it should be balanced, not absurdly overpowered or otherwise vulnerable to cheap exploitation. So that disqualifies the shotguns that shoot 30 rockets, the SMGs that shoot plasma grenades, needlers that fire a thousand rounds per second, etc. It should be readily incorporated alongside the standard weapons, or lend itself to an alternate type of gameplay (e.g., night vision). It does not have to be difficult to make. Sometimes simple or subtle mods are very effective.
Please include the author, map name, weapon name, a brief description of why you think the mod is superior, and whether it's a cosmetic or gameplay-oriented mod.
