problem with making map previews

alright i followed the tutorial and saved the final picture the right way. when i go to inject it into my main menu with main menu editor it comes up with an error message saying an unhandled exception occured. i press continue anyway and when i go to look at my maps, every map that didnt have a preview, has the same preview as the one i injected, so there are like 7 maps that i made with all the same map preview. please help!!

Apply that to a clean mainmenu
Add maps to it with a mainmenu editor, or however you wish
Inject preview images using Ch2r, or another map editor

hey i did this and when i inject with ch2r it says index outside the bounds of array. i saved the file correctly. the 8:8:8:8 is selceted and so is no mip maps, so i dont know what the deal is.
