Whenever I beat the Maw when Cortana says you need to get in the ship now and a target near the ship. When i reach the spot and it says 0 an exception error pops up

It's probably some sort of problem on your computer (hardware or software, not sure)
Recently, when my computer started acting like crap, I get those "exception" errors when I play online, at random usually. Could be adware, or hardware damage. (My mobo became the victem of ESD last year, but only recently showed signs of it)
