Is it a clean map, or have you already successfully injected a model into it? I followed the old Dark Matter tutorial too, and was getting the same error (in Dark Matter, not Entity) when I tried to inject another model into a map that I had already injected a new model into. Here's a pretty good tutorial that will tell you what you need to know. I use (and the tutorial uses) Xiso to make xbox .iso's, and I don't know anything about craxton, but as long as it makes an xbox .iso and not just an .iso, it should be fine.
